Transfer of Database

This form is to be used to transfer a microchip record from another UK compliant database to the Petlog database - the UK’s largest pet reunification service, managed by the Kennel Club. If you wish to transfer your pet from one compliant database onto Petlog, you can do this by applying here. Before proceeding, you must contact your pet's existing database for verification that your pet's microchip can be recorded with us. During this application, you must upload supporting documentation confirming the microchip can be transferred to the Petlog database.
